Caregiver Intake Form
The child’s caregiver completes an intake form — online, by phone, or by mail — providing caregiver, children’s, and parent information, plus signed permission forms.

FOR OUR FAMILIES FAMILY VISITATION PROGRAM
With prisons 100 miles or more away, most children never see their incarcerated parent. With Foreverfamily — they can.
Through our monthly prison trips, transportation is provided to Lee Arrendale and Pulaski State Prisons for Women so children can visit their mothers. We also partner with another agency so children can visit their fathers at select facilities.
Our children come from 3-generational homes and range from month-old infants to 18 years old. Prison trips run on the 2nd Saturday of every month — except January, May, June, and July. In May, we transport children on the first Saturday.
The roundtrip ranges from 3 to 6 hours. Children spend 5 hours with their mother in either the Children’s Center or General Visitation. Access to the Children’s Center is by application only — the mother must speak with the Children’s Center Director and have no disciplinary reports.
Trip Day Facts
Everything you need to know before your family's first visit. Our program site is located at 352 University Avenue, Atlanta, GA 30312.
Site Opens
5:00 AM — Doors Open
We provide a light breakfast for all children at the center before departure. Lunch and two snacks are provided on the bus during the trip.
Pulaski State Prison
Leaves 6:15 AM · Returns 5:30 PM
We pick up children in Fulton and DeKalb counties. Children outside these counties must be dropped off at Foreverfamily by an adult before the bus departs.
Children's Center Access
5 Hours with Mom Inside
Children spend 5 hours with their mother in the Children’s Center or General Visitation. The Children’s Center requires a clean disciplinary record and prior application by the parent.
Caregiver Attendance
Prison visits are primarily for children. Caregivers in the Children's Center program may attend twice per year:
- During Family Day — when siblings over 18 and other relatives are also welcome
- If the child is under 4 years old or has special needs or disabilities
Foreverfamily staff and trained volunteers always chaperone children on every trip. Your child will always be safe in our care.
